Simple studio glass vase by by Michael Rayner, co-founder of Island Glass (1972), which was later to become Alum Bay Glass (c.1982/83).

Jack in a pulpit is one of their signature shapes. The company was founded in 1972, and this isn't their current label. Other than that I don't know.
