It’s a slip molded vase in a style that’s a few decades older than the incised date; the original form would have been factory made and had the manufacturer’s stamp. The lack of a stamp, and the date, means it’s a craft kit piece, retailed to hobby potters. You quite often see the spotted green version of this glaze. They were very popular in the 60s-70s
